As a Partner Coach, you will be responsible for managing a multidisciplinary team of Partner Advocates, Peer Advocates, Nurse Advocates, and Resource Advocates who are working together providing care to and coordinating care on behalf of individuals facing substance exposed living. You will steward our innovative approaches to client engagement in close collaboration with community partners. Partner Coach Responsibilities
Lead and manage a team of Partner Advocates and Peer Advocates with the responsibility of overall team success.
Aid in the construction of clinical pathways, group therapy, classes and behavior focused practice protocols.
Conduct biopsychosocial assessments to determine behavioral health care treatment plans for patients.
Supervise and help manage complex cases that require a holistic clinical management approach
Provide evidence-based trauma-informed behavioral health care for patients.
Implement psychoeducation for patients during individual or group visits.
Provide same-day feedback to primary care provider and or care team.
Complete documentation for client contacts, touches, cancellations and care plan based on policies.
Responsible for achieving set goals/Key Performance Indicators (KPI's).
Expediting and supervising referrals to specialty behavioral health services, and when appropriate supporting a smooth transition from special behavioral health services to primary care.
Accountable to understanding role and how it affects utilization management benchmarks and quality outcomes.
Serve as a consultant to interdisciplinary team.
Participate in interdisciplinary team meetings providing clinical assessments and measures to coordinate treatment considerations.
Assist with the capturing program evaluation and fidelity measures.
Engaging in on-call crisis line management.
Promote responsible and ethical stewardship of the company's resources.
Greater than 25% travel (varies based on market maturity and may include occasional overnight stays).
Maintain excellent punctuality and attendance during work hours.

Partner Coach Qualifications Required:
Unrestricted Clinical Licensed Social Worker (LCSW) or Licensed Professional Counselor (LPC)
2+ years of clinical experience
1+ years management/supervision
Comprehension of the principles and practices of behavioral health treatment, including assessment, diagnosis, treatment planning, cognitive-behavioral and brief intervention counseling strategies and their application to population-based care for preventive and pervasive mental health issues
Skill and expertise in conducting group and individual psychoeducational classes and therapy strategies that will help strengthen the patient’s overall physical and mental health
Demonstrates the philosophy of the company through values and practices that integrate trauma informed care into client centered services
Reliable transportation and valid driver’s license
Basic computer proficiency; must be able to type and navigate a Cloud-based environment and designated teleconferencing application
2+ years of integrated behavioral health experience in a primary care setting.
Strong understanding/interest in public health and population health
Familiarity with Medicaid/Medicare/Commercial Insurance practices
A background in maternal/substance abuse populations
